WellFlorida, Rural Health Partnership and CommunityHealth IT specialize in:

  1. providing state-of-the art and highly secure health information exchange (HIE) services (community-, state-, and national-level)
  2. leveraging regional health information technology (HIT) resources
  3. performing HIT-related evaluations, trainings and consultations

HIE services are available through MyHealthStory. Developed by CommunityHealth IT and powered by RelayHealth, MyHealthStory is a personal health record for patients and an integrated longitudinal medical record with robust HIE tools for healthcare providers. This is the same portal platform used globally by the Department of Defense’s servicemen and servicewomen to access their health information electronically and the official portal platform for Tricare beneficiaries. It is also the mechanism for Florida Veterans to get their electronic health information to their civilian doctors.

MyHealthStory also enables providers and hospitals to electronically exchange protected health information in a way that satisfies Stage 1 and 2 Meaningful Use (e.g., how the federal government wants providers to use Electronic Health Records and perform HIE). These services empower both employer-sponsored health initiatives and patient consumerism by providing practical ways for residents to be more engaged in their healthcare through free technological tools. MyHealthStory is vendor and location agnostic, allowing access anywhere that providers and patients have Internet access, including smartphones.
