Coordinated Intake and Referral (CI&R) is a program supported by the Florida Maternal, Infant and Early Childhood Home Visiting Initiative (MIECHV) and provided by Healthy Start of North Central Florida Coalition. CI&R joins families with children ages 5 and under to services in Alachua County that aim to help families succeed. Initiated in 2016, the program is available to the public at UF Health-Pediatrics at the Children’s Medical Services (CMS) clinic and at the Mother Baby Unit at UF-Shands.

Carolina Rueda, the CI&R Family Partner, is establishing strong partnerships with key agencies that serve families with young children. Carolina has also been working alongside CMS and UF- Shands staff in order to generate a well-working CI&R process that will provide access to the support and services families need in order to be the best parents possible. These services include, but are not limited to, parenting education and support, childbirth and baby care education, counseling and family planning.

The CI&R program aims to provide home visiting services to more families with young children. This aim is propelled by the goal to ensure that families have the ongoing support they need to access parent education, services for children with special needs, mental health services for postpartum depression and various other issues. CI&R also provides the important job of connecting families to other parents with young children for mutual help and support during the early years of parenting.

The grant funding being used to implement this pilot project will be ending on September 30, 2017. Finding other resources or system changes to ensure the sustainability of the CI&R program past the end date of this funding is in pursuit.

CI&R is grateful to our current partners who are working together to connect families to the services they need. For potential participants to connect with a Family Partner, the newly created rack card is available below.
