Are you a first responder (firefighter, paramedic, emergency medical technician, law enforcement officer or public safety telecommunicator)? As a former or current first responder, your perspectives and experiences are important. This survey will be used to inform regional strategic planning efforts to address the mental health needs of first responders and their families. Please share your perspectives with us by completing this survey and by sharing it with other first responders in the northeast region of Florida.

The LSF Northeast Region First Responder Regional Support Project is funded by the Department of Children and Families with the intent to “provide regional supports targeting behavioral health needs of first responders and their families to reduce the incidence of suicide or attempted suicide among employed or retired first responders.” This survey is intended to better understand the needs of first responders and their families in the Northeast Region of Florida (Alachua, Baker, Bradford, Clay, Columbia, Dixie, Duval, Flagler, Gilchrist, Hamilton, Lafayette, Levy, Madison, Nassau, Putnam, Suwannee, St. Johns, Taylor, Union, and Volusia counties).

The first 100 first responders to complete the survey can receive a $50 Amazon electronic gift card. If you would like to receive a gift card, please provide a valid email address in the survey. Your contact information will only be used to distribute the gift card, it will not be shared. Your name and contact information will not be shared or published in the written report or provided to the Strategic Planning Committee.
